Become a member of our Jack’s Pack dining rewards program. It is easy and can pay for itself with our exclusive discounts. Just complete, sign and date, and return this application. We will invoice you for the $350 annual fee (plus Florida sales tax) with the first month of purchases.

Your Jack’s Pack membership is good for the full calendar year. We offer pro-rated memberships beginning in October at $30 (plus Florida sales tax) per month to receive the same great benefits.

Your membership benefits include:

• Ability to make advance reservations and request a seating area

• 10% off food and drink purchases

• 10% off purchases in our Marina Ships Store

Membership includes applicant and spouse/partner (if applicable). The undersigned applicant agrees to pay the membership fee (yearly or monthly), which will be included in the first month’s invoice, and all other charges to applicant’s account. Member must present a valid Jack’s Pack card and identification to receive discounts. All Jack’s Pack charges must be paid by ACH (no charge) or credit card (with 2.5% processing fee). All invoices are due upon receipt. A finance charge will be applied to all invoices unpaid after 30 days of date of invoice.
